Should you wish to establish with us, you may do so by completing the form(s) provided below and fax us your printed and completed form(s) or bring it with you to your appointment. If you have questions, contact us by telephone at 775.827.3033 and our staff will answer any queries you may have.
You need not of course establish until you wish to make an appointment but by doing so now you can stay informed with our pet-news service e-mail notification of breaking news and also very importantly of automatic notification of any infectious disease threats in your area which could affect your pet.
Furthermore if you tell us when your pet’s last vaccinations were we will send you a reminder (by e-mail or post) when their next annual health assessment & vaccinations are due so that you won’t forget!
